Forms and Documents
Visa Passport Pro will give you a check list of forms you must fill out and send in once you have selected and paid for a service. A common hold up of receiving a passport is incorrectly filling out a government form. This is where Visa Passport Pro helps. If you fill out a form incorrectly, we will contact you immediately to correct the mistake and have you resubmit
Make sure you read the checklist for your type of service below before filling out any forms. It is very important you do not make any mistakes.
Passport Forms are now available for download and printing. You must print these forms clearly on plain, 8.5" x 11" white paper with black ink.

New Adult Passport: Form DS-11
GET FORM DS-11
You must use this form if:
- You have never had a U.S. passport; and
- You are 16 years old or older
Passport Renewal: Form DS-82
GET FORM DS-82 You must use this form if you meet all of the following criteria:
- Your prior passport was issued when you were 16 or older.
- Your prior passport was issued within 15 years.
- Your prior passport is within your possession.
- Your prior passport is not mutilated or damaged.
Child Passport: Form DS-11
(DS-3053 Form is also needed in some cases, see below)
GET FORM DS-11 If your child is 15 years of age or under, please use the following form: DS11
IMPORTANTyou will also need to use the Statement of Consent Regarding a Minor (DS-3053) form if one of the legal guardians or parents cannot be present at the acceptance agent.
If your passport is Lost, Stolen or Damaged, you must fill out both the DS-11 and DS-64 forms
Name Change: Form DS-5504
GET FORM DS-5504 You must use this form if you need to change the name on your passport
this form only applies if your passport is less than a year old
Passport Pages: Form DS-4085
GET FORM DS-4085 You must use this form if you need to add pages to your passport
this form only applies if your passport has at least one year left of validity
- Your prior passport was issued within 15 years.
- Your prior passport is within your possession.
- Your prior passport is not mutilated or damaged.
