• Submit your valid, signed U.S. Passport, must have at least 6 months remaining
• 4 Application forms, fully completed and signed
• 4 Passport-type photographs required
• International Certificate of Vaccination for Yellow Fever
• Copy of round trip airline tickets or itinerary, or letter of confirmation from travel agent
• Postal Money Order made out to the consulate of Ghana for :

• Nature of business to be performed
• Name and address of reference to be visited
• Guarantee of return transportation and sufficient funds
• Letter is signed by company manager, other than applicant.
• Postal money order made out to the Consulate of Ghana for:
