• Submit your current valid and signed passport . It must not expire within 6 months and must have at least one blank visa page to apply for a 6 month or 1 year visa. If you wish to apply for a 5-10 year visa your passport must have Two blank pages that are back to back in the passport.

4. Page 2: At the bottom of Page 2, the applicant must sign. If applicant is a minor, both parents or legal guardians must sign.
